Job Summary:

We are seeking a highly skilled and compassionate Registered Practical Nurse (RPN) to join our team as a Care Coordinator (RPN) - Supervisor. As a key member of our care team, you will be responsible for providing exceptional care services to our residents, mentoring and guiding our care staff, and ensuring the highest standards of care are maintained.

Key Responsibilities:

  • Utilize the nursing process to provide high-quality nursing care to residents
  • Supervise and mentor care staff, providing guidance and education on an ongoing basis
  • Conduct assessments of residents moving into our community and follow up with residents throughout their transition
  • Establish priorities based on individual resident needs
  • Develop and implement person-centered service plans
  • Ensure compliance with resident assessments and service plans, following community-specific policies and regulations
  • Risk management through thorough monthly care-related audits
  • Administer medications, with knowledge of Point-Click-Care (PCC) an asset

Requirements:

  • Current Registered Practical Nurse license and good standing with the CNO
  • Current First Aid and CPR certification
  • Strong verbal and written communication skills
  • Ability to respond to emergency situations
  • Experience working with seniors in a retirement or long-term care facility environment an asset
  • Strong leadership and supervisory skills
  • PCC knowledge an asset
