Patient Care Coordinator

As a Registered Nurse with Alberta Health Services, you will play a vital role in delivering high-quality patient care while utilizing your critical thinking and problem-solving skills.
You will work collaboratively with healthcare teams to provide holistic and integrated care to patients, families, and communities.
With a focus on patient safety and well-being, you will apply nursing processes and best practices to ensure excellent care outcomes.
Key responsibilities include:
- Providing nursing care and services to patients with diverse needs and conditions.
- Collaborating with healthcare teams to develop and implement care plans.
- Utilizing critical thinking and problem-solving skills to address complex patient care issues.
- Communicating effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Alberta Health Services offers a competitive rate of pay, a remote retention allowance, and opportunities for professional growth and development.
Requirements include:
- Completion of an accredited nursing education program.
- Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
- Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.
Please note that provisional permit holders will be paid a rate of $35.87/hour - $43.82/hour.
