Patient Care Coordinator

We are seeking a compassionate Registered Practical Nurse (RPN) to join our healthcare team in Campbell River, Canada. In this role, you will provide quality nursing care to patients in various settings, working collaboratively with healthcare professionals to ensure coordinated care.
Key Responsibilities:- Assess patients' needs and develop individualized care plans.
- Administer medications and treatments as prescribed by physicians.
- Monitor patients' vital signs and report changes in condition.

- Provide education and support to patients and their families.
- Valid RPN license in Canada.
- Completion of a recognized Practical Nursing program.
- Previous experience in a healthcare setting is preferred.
- Strong communication and interpersonal skills.
