Social Services Professional

Vancouver Coastal Health is seeking a highly skilled Social Worker 3 to join their Long Term Care Team in Powell River, BC. This role involves providing clinical supports and consultation to all programs in the local area, as well as traveling to home visits, hospitals, clinics, and health centres to meet with staff, physicians, and clients.
Responsibilities- Provide consultation, education, mentorship, complex care planning, and clinical guidance within the context of a client- and family-centred care model.
- Develop decision support tools and policies, and recommend changes to practice based on current literature and national trends.
- Offer consultation, mentorship, education, and support to staff about all aspects of abuse, whether it be adult guardianship, adult protection, and/or domestic violence, as applicable to the designated portfolio.
- A Master's Degree in Social Work from an accredited University.
- Postgraduate continuing education training in working with vulnerable/incapable adults, including completion of the ReAct Adult Protection Curriculum (Modules 1-11).
- Five years' recent, related experience in working with issues of adult abuse and neglect and self-neglect with vulnerable adults, including recent social work experience working within the complex mental health and older adult populations.
- A Qualified Health Care Provider, as defined by AGA 2.1, and experience conducting assessments of incapability under Statutory Property Guardianship (AGA 2.1) and Health Care Consent and Care Facility Admissions Part 3 or an equivalent combination of education, training, and experience.
- Current full registration with the British Columbia College of Social Workers.
- Current BC Driver's License and access to a vehicle for travel within the region, as required.

The estimated salary for this position is $48.39-$59.19 per hour.
