About the Role

We are seeking a highly skilled Clinical Social Worker to join our team at Fraser Health Authority. As a Clinical Social Worker, you will play a vital role in providing therapeutic clinical counseling services to patients and families in our assigned clinical specialty area.

Key Responsibilities
  • Provide Basic Therapeutic Clinical Counseling Services: Explore the emotional response to identify problems, personal concerns, treatment objectives, and facilitate change in coping style, behavior, attitude, and feelings as it relates to the illness and other system issues.
  • Conduct Basic Social Work Assessments: Interview the patient and/or family, obtain relevant information, gather social data regarding the patient and/or family, and interpret the data using a variety of assessment tools, in accordance with professional standards, clinical policies, and practices.
  • Provide Consultation to Patients, Families, and Healthcare Professionals: Offer consultation on issues related to high-risk factors, recovery, and current community practice relevant to the assigned clinical specialty area, including adaptation to illness and disability, trauma, and bereavement.
  • Maintain Patient Records: Document patient assessments, charting information, prepare progress notes, and reports in accordance with established standards, policies, and procedures to meet regulatory requirements.
  • Provide Leadership in Discharge Planning: Interview the patient and/or family to assess and determine care plan and type of service required, articulate analysis and decision to members of the interdisciplinary team, and arrange family meetings to assist in developing and coordinating patient care plan and community services.
  • Communicate with Community Health and Social Agencies: Collaborate in the assessment and management of individuals at risk, make referrals to appropriate resources when longer-term treatment or follow-up is required, and exchange relevant information.
  • Act as a Resource to Patients and Families: Provide information for patients and families regarding community resources and access to these resources, suggest contacts to facilitate optimal planning, arrange referrals, and facilitate the development of community resources to support the health of the families.
  • Participate in Local Committees and Planning Bodies: Engage in program/practice evaluation, identify opportunities for change, assess available resources, initiate solutions, analyze outcomes, and identify areas for improved efficiency and effectiveness in service delivery to enhance patient care services and reduce service gaps.
  • Maintain Workload Statistics: Record information into approved formats, collate data, analyze trends, account for variances, prepare reports as required, and recommend an action plan to the Manager, Clinical Services, as required.
  • Attend and/or Lead Meetings, In-services, and Corporate Initiatives: Present relevant materials/topics to remain informed of departmental/practice activities and contribute to hospital initiatives.
  • Participate in Research Activities, Program Planning, and Development: Identify current trends/gaps in service, conduct literature searches, administer surveys, collect research data for further analysis, and ensure clinical practices and protocols are consistent with research findings.
  • Maintain Basic Development in the Social Work Profession: Review literature in the assigned clinical specialty area, consult with clinical and community experts within the field, evaluate clinical practice, and participate in professional development activities to improve care through new practices.
Qualifications

We are looking for a candidate with a Bachelor's degree in Social Work from an approved school of Social Work, plus one year recent, related acute care clinical experience. Current full registration with the BC College of Social Workers is also required.
