Social Worker

Fraser Health Authority is seeking a highly skilled and compassionate Social Worker - Clinical Specialist to join our team. As a key member of our interdisciplinary team, you will provide comprehensive social work services and therapeutic clinical counseling to patients and families in a hospital setting.
Key Responsibilities:- Conduct comprehensive social work and psychosocial assessments of patients and families
- Provide therapeutic clinical services, including grief and bereavement counseling
- Consult and educate patients, families, and healthcare professionals on high-risk factors, recovery, and current practices in the field of adaptation
- Maintain patient records and provide leadership to the interdisciplinary team in patient care planning
- Collaborate with community health and social agencies to determine alternate levels of care and effective utilization of patient resources
- Master's degree in Social Work from an approved school of Social Work
- Current full registration with the BC College of Social Workers
- Comprehensive knowledge and understanding of emotional and mental health issues related to illness, injury, and patient and family cultural dynamics
- Ability to educate and provide consultation to patients, families, and healthcare professionals
- Ability to develop, coordinate, and deliver in-services, presentations, psycho-educational, and support groups
