Clinical Social Worker

We are seeking a highly skilled Clinical Social Worker to join our team at Fraser Health Authority. As a Clinical Social Worker, you will play a vital role in providing comprehensive social work services and therapeutic clinical counseling to patients and families in our assigned clinical specialty area.
Key Responsibilities- Conduct Comprehensive Assessments: Conduct thorough social work and psychosocial assessments of patients and families, using a variety of assessment tools and in accordance with professional standards, clinical policies, and practices.
- Provide Therapeutic Services: Provide comprehensive therapeutic clinical services, including grief and bereavement counseling, to patients and families in the assigned clinical specialty area.
- Consult and Educate: Provide consultation and education to patients, families, and other healthcare professionals on issues related to high-risk factors, recovery, and current practice in the field of adaptation.
- Document Patient Information: Maintain accurate and up-to-date patient records, documenting patient assessments, charting information, and preparing progress notes and reports in accordance with established standards, policies, and procedures.
- Lead Interdisciplinary Teams: Provide leadership to the interdisciplinary team in patient care planning, including discharge planning and hospital stay, by articulating analysis and decision to members of the team.
- Collaborate with Community Agencies: Communicate with community health and social agencies, including provincial Ministries, Home Health personnel, and family physicians, to assess and manage individuals at risk and make referrals to appropriate resources.
- Develop and Deliver In-Services: Develop, coordinate, and deliver in-services, presentations, psycho-educational and support groups, and training to healthcare professionals and patients.
- Initiate and Develop Research: Initiate, develop, and carry out research activities and program planning and development relevant to the needs of patients, families, hospital, and community.
- Education and Experience: Master's degree in Social Work from an approved school of Social Work, plus one (1) year recent, related acute care clinical experience.
- Registration: Current full registration with the BC College of Social Workers.
- Skills and Abilities: Comprehensive knowledge and understanding of emotional and mental health issues related to illness, injury, and patient and family cultural dynamics; comprehensive counseling skills, including crisis counseling, brief therapeutic interventions, and loss and grief applicable to patient, families, and their support networks.
