Clinical Social Worker

We are seeking a highly skilled Clinical Social Worker to join our team at Fraser Health. As a key member of our interdisciplinary team, you will provide high-quality therapeutic clinical counseling services to patients and families in our assigned clinical specialty area.
Key Responsibilities- Conduct comprehensive social work assessments of patients and families using a variety of assessment tools, in accordance with professional standards and clinical policies.
- Provide consultation to patients, families, and other healthcare professionals on issues related to high-risk factors, recovery, and current community practice.
- Develop and coordinate patient care plans, including arranging family meetings and collaborating with hospital and community services to determine alternate levels of care.
- Communicate effectively with community health and social agencies, including government ministries, home health personnel, and family physicians.
- Act as a resource to patients and their families, providing information on community resources and access to these resources.
- Bachelor's degree in Social Work from an approved school of Social Work.
- Current full registration with the BC College of Social Workers.
- One year recent, related acute care clinical experience.
