Job Title: Social Worker - Clinical Specialist

Fraser Health Authority is seeking a skilled Social Worker to join our clinical team. As a Social Worker - Clinical Specialist, you will provide comprehensive social work services and therapeutic clinical counseling to patients and families in a hospital setting.

Key Responsibilities:
  • Conduct comprehensive social work and psychosocial assessments of patients and families.
  • Provide therapeutic clinical services, including grief and bereavement counseling.
  • Consult and educate patients, families, and healthcare professionals on high-risk factors, recovery, and current practices in adaptation.
  • Maintain patient records and document patient information.
  • Provide leadership to the interdisciplinary team in patient care planning.
  • Communicate with community health and social agencies to facilitate referrals and resource utilization.
Requirements:
  • Master's degree in Social Work from an approved school.
  • Current full registration with the BC College of Social Workers.
  • Comprehensive knowledge and understanding of emotional and mental health issues.
  • Ability to educate and provide consultation to patients, families, and healthcare professionals.
  • Ability to develop, coordinate, and deliver in-services, presentations, and psycho-educational groups.
