Job Summary

We are seeking a highly skilled and compassionate Social Worker to join our Inpatient Mental Health team at Southlake Regional Health Centre. As a key member of our interdisciplinary team, you will provide assessment and support to individuals with mental health needs, working collaboratively with patients, families, and healthcare professionals to ensure the best possible outcomes.

Key Responsibilities
  • Conduct comprehensive assessments of patients' mental health needs, identifying areas for support and intervention.
  • Develop and implement individualized treatment plans, working closely with patients and their families to achieve goals and improve mental health outcomes.
  • Provide short-term, supportive counseling and education to patients and their families, promoting empowerment and self-management of mental health.
  • Collaborate with community partners to ensure seamless transitions and referrals, ensuring patients receive the support they need to thrive.
  • Participate in interdisciplinary team meetings, contributing to the development of comprehensive care plans and ensuring patient-centered care.
  • Document patient progress, treatment plans, and outcomes, maintaining accurate and up-to-date records.
Qualifications

To be successful in this role, you will possess a Master's degree in Social Work (MSW) and a minimum of three years' experience working with seriously mentally ill clients. You will also have advanced counseling and assessment skills, with experience in Brief Solution Focused model, cognitive behavioral, and dialectical behavioral therapy. Strong communication and interpersonal skills are essential, as is the ability to work collaboratively in an inter-professional team environment.
