Job Summary

We are seeking a skilled Social Worker to join our team. The ideal candidate will have a Master's degree in Social Work from an accredited university and previous experience relating to patients with cardiovascular disease.

Previous experience in Chronic Disease Management and Cardiac Rehabilitation settings is an asset. Membership in the Ontario College of Social Workers and Social Service Workers is required.

  • Develop and implement patient-centered care plans to support individuals with complex healthcare needs.
  • Collaborate with healthcare teams to provide comprehensive care and support services.
  • Provide education and support to patients and families to promote healthy behaviors and disease management.
  • Conduct assessments and develop plans to address social determinants of health and promote patient well-being.
