Job Summary

We are seeking a highly skilled and compassionate Social Worker to join our Inpatient Mental Health team at Southlake Regional Health Centre. As a Social Worker, you will play a vital role in providing assessment and support to individuals with mental health needs, working collaboratively with a multidisciplinary team to ensure the best possible outcomes for our patients.

Key Responsibilities
  • Conduct individual and family assessments to determine the type of services required for successful discharge
  • Provide supportive counseling and education to individuals and their families
  • Develop and implement treatment plans in collaboration with the treatment team
  • Facilitate referrals to community resources and support services
  • Participate in daily inpatient rounds and contribute to the development of a comprehensive care plan
  • Collaborate with community partners to ensure seamless transitions and continuity of care
Requirements
  • Masters in Social Work (MSW) from an accredited program
  • Three to five years' experience working with seriously mentally ill clients
  • Ontario College of Social Workers and Social Service Workers (OCSWSSW) registration
  • Advanced counseling and assessment skills, including brief solution-focused model, cognitive behavioral, and dialectical behavioral therapy
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment
  • Ability to work collaboratively as part of a multidisciplinary team
