Job Summary

We are seeking a skilled Social Worker to join our team at Southlake Regional Health Centre. As a member of our inter-professional Medicine team, you will provide assessment, treatment, consultation, and education services to patients and their families.

Key Responsibilities
  • Provide short-term crisis and grief counselling for patients, families, and team members.
  • Counsel patients and families regarding hospital admission and discharge to provide continuity of care.
  • Maintain documentation of clinical assessments, treatment plans, and patient clinical progress.
  • Act as a resource and liaison to community stakeholders and facilitate referrals to community resources.
  • Link patients, families, and/or significant others to support in the community.
Requirements
  • Masters of Social Work from an accredited university.
  • Membership in the Ontario College of Social Workers and Social Service Workers required.
