Job Title: Clinical Care Specialist

At Hamilton Health Sciences, we are seeking a skilled Clinical Care Specialist to join our team of healthcare professionals.

Key Responsibilities:

  • Provide high-quality rehabilitative care to patients with diverse disabilities.
  • Collaborate with multidisciplinary staff to deliver comprehensive care.
  • Evaluate patient progress and adjust treatment plans accordingly.

Requirements:

  • Degree in Nursing from an accredited institution.
  • Current Certificate of Registration with the College of Nursing of Ontario.
  • Recent experience working with rehabilitation or medical patients.
  • Excellent communication and interpersonal skills.
  • Demonstrated knowledge of Rehabilitation nursing principles.
  • Commitment to interdisciplinary care and a nursing professional practice model.

Estimated Salary Range: $85,000 - $115,000 per year (based on national averages).
