Clinical Wound Specialist

We are seeking a skilled Clinical Wound Specialist to join our team at SE Health. This is a part-time or full-time opportunity in Hamilton, with no weekend work required. As a member of the Wound Ostomy Continence (WOC) Team, you will provide specialized care to individuals and families with ostomies, acute and chronic wounds, and urinary and fecal continence concerns.

About the Role
In this role, you will formulate treatment recommendations in collaboration with the interprofessional team, develop a plan of care based on holistic assessment for clients requiring wound, ostomy, and/or continence care. You will evaluate the plan of care, involve the Worn and/or K2P WCRN in the evaluation process as appropriate. Your responsibilities will also include providing education and mentorship experiences to build wound, ostomy and continence knowledge and capacity within SE Health. Furthermore, you will provide consultation to care providers within the organization, coordinate/collaborate with the interprofessional team across the continuum of care to reduce duplication of services, client anxiety, and cost.
Requirements
To be successful in this role, you must have completed the Nurses Specialized in Wound, Ostomy & Continence Canada (NSWOCC) WOC-EP program (international certification will be considered if approved by NSWOCC). Flexibility and a commitment to advancing wound, ostomy, and continence care at SE Health is required. A willingness to participate in further education programs/courses to remain current is essential. Additionally, adoption of the SE model for wound care delivery through mentorship of WORN/K2P WCRN, a vehicle and valid driver's license, and a willingness to travel are required. A comprehensive knowledge of community health care is preferred, as well as an ability to use computer word processing, PowerPoint, email, and source information on the internet. The estimated annual salary for this role is approximately $120,000 - $140,000 depending on experience.
