About the Role

We are seeking a skilled Registered Practical Nurse to join our team in a community nursing role. As a RPN, you will provide high-quality care to patients in their homes, working collaboratively with RNs to ensure comprehensive care.

Key Responsibilities
  • Participate in scheduled visits to assess patient needs and provide care within your scope of practice
  • Develop and implement individualized care plans, monitoring patient progress and adjusting as needed
  • Engage patients and their families in the care process, promoting independence and empowerment
  • Maintain accurate and up-to-date patient records, ensuring seamless communication with healthcare teams
Requirements
  • Registration with the CNO as a Registered Practical Nurse in good standing
  • Valid driver's license and access to a vehicle
  • Experience in community care is an asset, but not required
  • Reliability, flexibility, and a passion for making a difference in your community
