Community Care Nurse

About the Role:
In this community nursing position, you will provide scheduled visits to patients in various settings, working closely with RNs for more complex cases. Your role will involve caring for patients of all ages with diverse care needs, helping them remain comfortable in their homes for as long as possible.
Key Responsibilities:
- Participate in scheduled visits to assess patient needs and provide care within your scope of practice
- Deliver safe and ethical nursing care, using appropriate assessment tools and techniques
- Develop and continuously monitor care plans, adjusting as required
- Engage patients using a collaborative approach, building rapport and trust
- Maintain and update professional patient records related to patient encounters and plans
Requirements:
- Registration with the CNO as a Registered Practical Nurse in good standing
- Valid driver's license and access to a vehicle
- Experience in delivering community care is an asset
- Reliability and flexibility
- Willingness to travel to and from patient residences within a designated area
- Experience working with vulnerable populations
- Passion for making a difference in your community
