About the Role:

We are seeking a skilled Registered Practical Nurse to join our team in the community setting. As a Community Care Nurse, you will have the opportunity to provide high-quality care to patients in their homes, working collaboratively with our team to ensure the best possible outcomes.

Key Responsibilities:

  • Participate in scheduled visits to assess patient needs and provide care within your scope of practice
  • Provide safe and ethical nursing care, using appropriate assessment tools and techniques to assess patients' individual needs
  • Execute and continuously monitor care plans, adjusting as required to ensure optimal patient outcomes
  • Engage patients using a collaborative approach, building rapport and trust to promote positive health outcomes
  • Maintain and update professional patient records related to patient encounters and plans

Requirements:

  • Registration with the CNO as a Registered Practical Nurse in good standing
  • Valid driver's license and access to a vehicle
  • Experience in delivering community care is an asset
  • Reliability and flexibility
  • Willingness to travel to and from patient residences within a designated area
  • Experience working with vulnerable populations
  • Passion for making a difference in your community
