Community Care Nurse

About the Role:
We are seeking a skilled Registered Practical Nurse to join our team in the community setting. As a Community Care Nurse, you will have the opportunity to provide high-quality care to patients in their homes, working collaboratively with our team to ensure the best possible outcomes.
Key Responsibilities:
- Participate in scheduled visits to assess patient needs and provide care within your scope of practice
- Provide safe and ethical nursing care, using appropriate assessment tools and techniques to assess patients' individual needs
- Develop and implement care plans, continuously monitoring and adjusting as required
- Engage patients using a collaborative approach, building rapport and trust to promote positive outcomes
- Maintain and update professional patient records related to patient encounters and plans
Requirements:
- Registration with the CNO as a Registered Practical Nurse in good standing
- Valid driver's license and access to a vehicle
- Experience in delivering community care is an asset
- Reliability and flexibility
- Willingness to travel to and from patient residences within a designated area
- Experience working with vulnerable populations
- Passion for making a difference in your community
