Nurse Care Coordinator

As a key member of our healthcare team, you will be responsible for the care and well-being of our residents, working under the direction of the Resident Services Manager. You will provide guidance and support to attendants and unregulated care providers, ensuring that our residents receive the highest level of care.
Qualifications and Requirements
A registered practical nurse with a current Ontario Certificate of Competence from the College of Nurses of Ontario is required. You must be able to follow oral and written instructions in the English language and possess a medication administration certificate. Your excellent communication and interpersonal skills will enable you to work effectively with our residents, families, and healthcare team.
Key Responsibilities
Participate in our Quality Improvement Program, assessing the needs of individual residents and organizing care to promote maximum independence. Carry out planned assignments and job routines, taking into account the physical, spiritual, social, and emotional needs of each resident. Supervise the nursing care given by attendants and unregulated care providers, ensuring that care plans are followed. Provide direct resident care, assisting with the implementation and enforcement of our policies. Maintain accurate records of nursing observations and procedures performed. Participate in the development and implementation of care plans, supporting independence as a member of our multi-disciplinary team. Support and participate in unit orientation and complete applicable educational training. Administer medication in accordance with the College of Nurses standards and our policies. Consult with residents, families, and physicians regarding service interventions and referrals. Record all charges for services on individual resident sheets. Comply with the Occupational Health and Safety Act, Workplace Safety and Insurance Act, and Workplace Hazardous Material Information System. Perform other related duties as delegated by the Resident Services Manager.
