We are seeking a skilled Health Care Social Worker to join our team at Nova Scotia Health and IWK Health. As a member of our interprofessional team, you will play a vital role in delivering Primary Health Care and chronic disease management services, with a focus on chronic pain.
The successful candidate will be responsible for building relationships with patients and families, assessing their needs, and developing and implementing plans to address their psychosocial, emotional, and spiritual well-being. You will work collaboratively with an interdisciplinary team to provide comprehensive care and support.
We are looking for a candidate with a Master's degree in social work from a recognized university, current registration with the Nova Scotia College of Social Workers, and at least 2 years of post-MSW clinical experience. Previous experience working in Primary Care and/or community-based settings is an asset.
The ideal candidate will possess excellent communication skills, the ability to work independently and collaboratively, and a commitment to diversity, equity, and inclusion. You will also have the ability to work flexibly and creatively to provide services, and be comfortable working with an interdisciplinary team and community partners.
We offer a competitive salary of $49.04 per hour, as well as a comprehensive benefits package, including health, dental, travel, long-term disability, and life insurance coverage, and a defined benefit pension plan.
