Job Summary

We are seeking an experienced End-of-Life Care Specialist to join our team at Vancouver Coastal Health. The successful candidate will be responsible for providing specialized care coordination and support to patients, families, and healthcare staff.



Key Responsibilities


  • Provide comprehensive care planning and supportive counseling to patients and their families.
  • Collaborate with interdisciplinary care teams to ensure seamless delivery of care.
  • Develop and implement trauma-informed, culturally safe, and strengths-based care approaches.
  • Promote excellence in patient-centered care, education, and research.
  • Act as a clinical resource and provide consultation support to healthcare staff.


Requirements


  • Degree in Social Work or related field from an accredited institution.
  • Minimum 3 years of experience in acute, facility, or community settings supporting end-of-life care.
  • Valid BC Driver's License and reliable transportation.


Compensation

The salary range for this position is CAD $42.27/Hr. - CAD $52.81/Hr.
