Vancouver Coastal Health is seeking a skilled Registered Nurse to join our Post-Transplant team at Vancouver General Hospital.
Job SummaryThis is an exciting opportunity for a dedicated and compassionate nurse to work with our interdisciplinary team, providing exceptional care to clients post-solid organ transplantation.
Responsibilities- Assess and coordinate client care, working collaboratively with the healthcare team to ensure seamless transitions throughout the client's lifespan.
- Develop and implement comprehensive education plans using evidence-based principles and adult learning theories.
- Participate in multidisciplinary committees, contributing expertise and knowledge to optimize client outcomes.
- Stay current with research and literature, applying new knowledge to inform practice and improve nursing standards.
To be successful in this role, you will have:
- A valid practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Completion of two years' recent, related experience, including one year in adult patient education or an equivalent combination of education, training, and experience.
- Knowledge of nursing theory and practice within a client-centered model of care.
- Familiarity with BCCNM standards of practice for registered nurses.
- Understanding of adult education principles and methods.
- Knowledge of evidence-based clinical practice related to solid organ transplantation.
- Ability to function as a team member, leader, and facilitator.
- Excellent communication and interpersonal skills.
