We have an exciting opportunity for a Senior Clinical Nurse Specialist to join our team in Supported Housing As a Registered Psychiatric Nurse or Registered Nurse, you will work with a dedicated multi-disciplinary team to provide care tailored to clients' complex medical and/or cognitive needs.

About the Role

This position involves:

  • Managing a caseload of eligible clients who are chronically ill or have long-term physical disabilities;
  • Using chronic disease management and self-care management principles to conduct assessments, identify problems, address issues, or variances from the plan of care in collaboration with the appropriate physician, interdisciplinary team member, and/or community agency;
  • Providing continuity of care across the continuum by liaising with interdisciplinary team members, coordinating, implementing clinical interventions, evaluating client outcomes based on client goals for care, coordinating ongoing care, and supporting transition;
  • Acting as a clinical resource for the interdisciplinary team by providing information based on nursing theory and practice related to clients with complex healthcare needs;
  • Consulting, conferring, and collaborating with multidisciplinary team members, physicians, educational resources, and other healthcare professionals to achieve excellence in client-centred education and research;

About You

We are looking for a highly skilled and experienced Registered Psychiatric Nurse or Registered Nurse with a strong background in acute care and case management. You should have excellent communication and leadership skills, as well as the ability to work independently and prioritize care for a caseload of clients.

Estimated Salary Range

The estimated salary range for this position is between $45,000 and $65,000 per year, depending on experience.
