At Vancouver Coastal Health, we are seeking a skilled and dedicated Healthcare Transition Specialist to join our team in Vancouver. This role offers an exciting opportunity for a Registered Nurse to lead and facilitate acute to community transitions by assessing and evaluating clinical and functional need and coordinating community staff/resources in carrying out the care following discharge from an acute site.

This position is responsible for developing and implementing effective policies, procedures, and standards for the transition services program in community care. The successful candidate will also provide coaching and encouragement to acute and community colleagues to embrace new and evolving clinical pathways and practice. Additionally, they will plan and facilitate clinical discussions to support safe client discharges with care teams and other healthcare professionals.

We are looking for a candidate with strong analytical and problem-solving skills, as well as excellent communication and interpersonal skills. The ideal candidate will have a solid understanding of nursing theory and practice within a client and family-centered model of care. They will also possess broad knowledge of evidence-based nursing practice related to patient acuity and bed utilization practices within a tertiary level teaching hospital.

The estimated salary for this position is CAD $51,500-$64,300 per year, based on the national average salary range for a Registered Nurse in Vancouver, BC.
