Job Description

VON Canada (Ontario) is seeking a skilled Community Care Leadership Professional to join our team. This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying, bending, reaching, kneeling, and other movements that emphasize good body mechanics.

The successful candidate will have a Post-Secondary education in a related field, Registered Nurse (RN) or Registered Practice Nurse (RPN) with current registration with CNO, and a minimum of 5 years experience in community healthcare and a minimum of 2 years experience managing staff.

This is a full-time position with an estimated annual salary of $85,000 - $110,000 depending on qualifications and experience. The ideal candidate will have excellent critical thinking, assessment, and problem-solving skills, strong attention to detail, and excellent verbal and written communication skills.

Key Responsibilities
  • Directs and monitors the delivery of client care services and day-to-day functions and performance of front-line staff in accordance with current regulatory guidelines and VON policies, procedures, and standards.
  • Assists staff, or completes risk assessments, investigations of issues, injuries, and reporting related to client or employee incidents.
  • Monitors performance related to funder expectations regularly and contributes to the development and implementation of quality improvement activities to achieve performance targets through monitoring of reported data.
Requirements
  • Post-Secondary education in a related field.
  • Registered Nurse (RN) or Registered Practice Nurse (RPN) with current registration with CNO.
  • A minimum of 5 years experience in community healthcare and a minimum of 2 years experience managing staff.
