Job Summary:

The Community Care Manager (HCC) RN/RPN provides strategic leadership to staff and ensures the delivery of high-quality client care within their program area(s). The position is accountable for working as part of an integrated management team to ensure operational goals are met.

Key Responsibilities:

  • Directs and monitors the delivery of client care services, day-to-day functions, and performance of front-line staff in accordance with current regulatory guidelines and VON policies.
  • Assists staff or completes risk assessments, investigations of issues, injuries, and reporting related to clients or employees.
  • Monitors performance related to funder expectations regularly and contributes to the development and implementation of quality improvement activities to achieve performance targets.
  • Guides new employees through the orientation process and ensures all probationary reviews and stay interviews are completed.
  • Applies knowledge of collective agreements and local labor rules in daily decision-making.

Education, Designations, and Experience:

  • Post-secondary education in a related field.
  • Registered Nurse (RN) or Registered Practice Nurse (RPN) with current registration with CNO.
  • A minimum of 5 years of experience in community health care and a minimum of 2 years of experience managing.

Estimated Salary Range: $85,000 - $115,000 per annum, depending on qualifications and experience.
