About the Opportunity

The Care Coordinator is responsible for assessing, planning, and authorizing services for clients referred to Continuing Care. They help provide an array of services to assist individuals and families cope with complicated health situations in the community.

Care Coordinators assist clients to identify their goals, unmet needs, and resource requirements. Based on the assessment, they assist the client and their care partners to formulate a plan to meet identified goals.

Care Coordinators work in the community, hospital, or long-term care facilities. They are fiscally responsible for the care, services, and equipment they authorize within available Department of Seniors and Long Term Care policies and programs.

Care Coordinators maintain ongoing communication with clients, family, caregivers, care providers, and healthcare teams within the client's identified circle of care. The philosophy of the Care Coordinators is grounded in 'Home First': maximizing available services and resources to support clients and families in the home/community.

Care Coordinators are responsible for reassessments at routine intervals and skilled care management.

About You

We are seeking a qualified professional with a Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies. The ideal candidate will have a minimum of three years' experience with the professional regulatory body and demonstrated knowledge and/or experience in a community or long-term care setting.

The successful candidate will possess excellent assessment, interviewing, and negotiation skills, as well as excellent verbal and written communication skills. They will be able to work independently and co-operatively in a busy multidisciplinary situation and prioritize workload and manage time efficiently.

Valid driver's license and access to a reliable vehicle are required. Competencies in other languages are preferred, with French being an asset.

Hours

This is a full-time, long-assignment position with 70 hours bi-weekly. The start date is as soon as possible, and the end date is December 2025. Shifts may include days, evenings, weekends, and holidays.

Compensation and Benefits

The hourly rate is $41.06 - $46.75. Successful candidates may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
