Job Summary

We are seeking a skilled and experienced Registered Nurse to join our team as a Transition Care Specialist. This is a challenging and rewarding role that requires strong clinical skills, effective communication, and a patient-centered approach.

Key Responsibilities
  • Conduct comprehensive assessments of patients' needs and develop individualized care plans
  • Collaborate with healthcare teams to coordinate care and services across the continuum
  • Provide education and support to patients, families, and caregivers
  • Develop and maintain relationships with community resources and healthcare providers
  • Participate in quality improvement initiatives and contribute to a culture of excellence
Requirements
  • Current registration with the College of Registered Nurses of Alberta (CRNA)
  • Minimum of 5 years of facility-based nursing experience, including geriatric experience
  • Minimum of 2 years of experience in community nursing (home care, supportive living, or facility living)
  • Effective time management and prioritization skills
  • Strong communication and conflict resolution skills
  • Ability to work independently and as part of a team
Preferred Qualifications
  • Knowledge of computer applications such as Microsoft Word, Pathways, RAI Tools, Meditech, and e-People
  • Active Transition Coordinators with up-to-date certification may be given preference
