Indigenous Health Liaison Specialist

Fraser Health Authority is seeking a dedicated and culturally sensitive Indigenous Health Liaison Social Worker to join our team. As an integral member of our multidisciplinary team, you will play a crucial role in enhancing service delivery for First Nations, Metis, and Inuit clients.
About the RoleThis is an exciting opportunity for a skilled social worker to make a meaningful difference in the lives of Indigenous individuals and families. Reporting to the Clinical Lead, Indigenous Health Liaison Team, you will work collaboratively with community partners and healthcare professionals to provide culturally appropriate counseling and intermediary services.
Your key responsibilities will include:
- Providing culturally sensitive counseling and support to Indigenous clients and their families;
- Assessing and planning clinical and psychosocial health care needs and services;
- Liaising with community agencies and healthcare providers to ensure seamless care coordination;
- Developing and implementing health education programs and initiatives;
- Maintaining accurate records and reports to track client progress and outcomes.
