About the Job
We are seeking a Community Transition Nurse Lead to join our team. This role will play a critical part in ensuring seamless transitions between correctional facilities and community programs, providing support to individuals with complex mental health and substance use challenges.
Job Summary
This position requires a Registered Nurse or Registered Psychiatric Nurse with current practicing registration as a RN or RPN with the British Columbia College of Nurses and Midwives (BCCNM). The ideal candidate will have two years of recent, related experience in acute or primary care or community health nursing, or an equivalent combination of education and experience.
The estimated salary for this position is $83,000 - $120,000 per year, depending on qualifications and experience.
Duties and Responsibilities
- Regularly communicate with other members of the health care interdisciplinary team regarding anticipated release of clients and their potential needs for transition support;
- Work with inter-professional health care treatment teams, BC Corrections staff, clients, and family members to validate demographic data and health information and determine nature and extent of needs for community health substance use support services;
- Liaise with key community resources and ensure accurate and complete written information transfer to programs and services across the province;
- Facilitate linkages to community programs, Regional Health Authority services, community health services, substance use services, and other community support services to establish clear transition plans;
- Engage in ongoing team planning, development, evaluation, and quality improvement activities that facilitate team goal setting, problem solving, and conflict resolution.
Required Skills and Qualifications
- Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse or Registered Psychiatric Nurse;
- Two years recent, related experience in acute or primary care or community health nursing or an equivalent combination of education and experience;
- Develop partnerships and relationships with external partners to ensure continuity of client care;
- Apply and evaluate nursing theory and expertise;
- Understand services and philosophy of primary health care;
- Assess need for and initiate use of community services;
- Communicate effectively, both verbally and in writing;
- Commitment to develop knowledge and understanding of legislative obligations and provincial commitments found in foundational documents.
