Community Support Specialist

We are seeking a highly motivated and compassionate Community Support Worker to join our team at NEXUS Community Support Society.
Key Responsibilities- Work collaboratively with individuals and their support networks to develop and implement personalized plans.
- Facilitate community inclusion and participation in various settings, promoting social connections and a sense of belonging.
- Conduct regular documentation and record-keeping to track individual progress and identify areas for further support.
- Participate in team meetings, training sessions, and service planning to ensure effective collaboration and communication.
- Hold a valid Class 5 driver's license and be willing to use it for work-related purposes.
- Related experience working with adults with diverse abilities is considered an asset.
- Basic computer skills and physical ability to perform job duties.
- First Aid certification (Level 1).
