Community Support Specialist

NEXUS Community Support Society is seeking a dedicated and compassionate Community Support Worker to join our team. As a Community Support Worker, you will play a vital role in supporting individuals with diverse abilities to live fulfilling lives in their communities.
Key Responsibilities- Develop and implement personalized support plans to meet the unique needs and goals of individuals supported by NEXUS.
- Facilitate community inclusion and participation in various settings, promoting social connections and a sense of belonging.
- Collaborate with individuals, families, and support networks to ensure seamless support and services.
- Document and maintain accurate records of individual progress, successes, and areas for further support.
- Participate in training opportunities, meetings, and service planning to enhance skills and knowledge.
- Must be 19 years of age or older.
- Hold a valid class 5 (or better) driver's license – driver's abstract required.
- Related experience working with adults with diverse abilities is considered an asset.
- Basic computer skills and physical ability to carry out duties as required.
- First Aid (Level 1)
- Criminal record check (will be performed by NEXUS upon hire)
