About the Opportunity

**Job Summary**

The Nova Scotia Health Authority is seeking a highly skilled and compassionate Community Outreach Worker to join our team. As a Community Outreach Worker, you will provide a range of clinical services to adults and their families affected by mental illness and/or substance use or gambling disorder.

**Key Responsibilities**

  • Provide clinical services in both clinic and community settings, including screening, assessment, brief intervention, case management/navigation, individual and group treatment.
  • Work with individuals who are unhoused, living in encampments, and provide direct support and navigation services.
  • Develop and maintain relationships with community organizations and agencies to coordinate resources and services.
  • Apply principles of harm reduction, community development, and psychosocial rehabilitation in your practice.
  • Work collaboratively with colleagues, clients, and family members to create and enrich linkages within the community.

**Requirements**

  • Bachelor of Social Work degree.
  • Registration or eligibility for registration with a professional regulatory body.
  • One year post-BSW clinical experience in Mental Health and Addictions, with a Master's degree considered an asset.
  • Screening and risk assessment skills, including suicide risk assessment.
  • Ability to work independently, assess situations, and implement evidence-based plans of care.
  • Working knowledge of the DSM.
  • Experience in offering brief, supportive intervention strategies focusing on harm reduction and recovery.
  • Ability to navigate and de-escalate situations, and offer crisis support.
  • Experience and/or demonstrated understanding of the barriers faced by the unhoused community.
  • Group facilitation skills.
  • Ability to use motivational interviewing skills and engage with clients/families.
  • Demonstrated knowledge of community resources, program development methods, and recovery-oriented practice.
  • Knowledge of social determinants of health and its importance to decision formulation.
  • Knowledge and experience with a trauma-informed care approach.
  • Excellent communication and interpersonal skills.
  • Ability to build and maintain collaborative working relationships.
  • Strong problem-solving skills.
  • Demonstrated ability to make sound and reliable clinical decisions.
  • Competencies in other languages an asset, with French preferred.
  • Access to a reliable vehicle for travel.

**What We Offer**

  • Casual full-time assignment with 75 hours biweekly.
  • Approximately 1-year term.
  • 65% of this position requires travel to provide outreach within the community.

**Compensation and Incentives**

$42.01 hourly.
