Clinical Social Worker

We are seeking a skilled Clinical Social Worker to join our team at Fraser Health. As a key member of our interdisciplinary team, you will provide high-quality social work services to patients and families, promoting optimal functioning and mastery of situations.
Key Responsibilities- Conduct thorough social and psychosocial assessments of patients and families, interpreting information using a variety of modalities in accordance with professional standards and clinical practice.
- Provide therapeutic counseling services to clients and families experiencing a range of circumstances related to their illness, promoting client/family autonomy and problem-solving abilities.
- Facilitate the development of comprehensive care plans with/for clients, collaborating with other community professionals and healthcare disciplines.
- Act as a resource for clients and families, providing information regarding community resources and access to these resources, facilitating referrals where appropriate.
- Participate in quality improvement activities, providing input into the development, implementation, and evaluation of program goals, objectives, policies, and procedures.
To be successful in this role, you will require:
- A Bachelor's degree in Social Work from an approved school of Social Work, plus two years' recent related clinical social work experience working with individuals and families with mental health and substance use issues.
- Current full registration with the BC College of Social Workers.
- Valid BC Driver's License and access to a personal vehicle for business-related purposes.
You will possess:
- Demonstrated knowledge and understanding of counselling skills and clinical practice models as it relates to individuals, families, and groups.
- Ability to conduct social and psychosocial assessments, provide leadership to the interdisciplinary team in care planning, and work effectively in a multicultural environment.
- Strong communication and collaboration skills, with the ability to facilitate group meetings, processes, and decision-making.
