$22.612 - $23.291 per hour

Job Description

We are seeking an experienced Care Coordinator to join our team at the Grace Hospital Hospice. As a key member of our interdisciplinary team, you will play a vital role in providing compassionate and coordinated care to our patients and their families.

Your primary responsibilities will include:
  • Providing administrative support to the hospice team, including maintaining accurate records and reports.
  • Collaborating with healthcare professionals to develop and implement individualized care plans.
  • Communicating effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Maintaining a safe and welcoming environment for patients and staff.

Required Skills and Qualifications

To be successful in this role, you will require:
  • A Grade 12 education or equivalent combination of education and work experience.
  • Experience working in a healthcare setting, preferably in a hospice or palliative care environment.
  • Strong communication and interpersonal skills, with the ability to build rapport with patients, families, and healthcare teams.
  • Demonstrated ability to work collaboratively as part of a dynamic team.
  • Basic computer skills and proficiency in software programs such as Microsoft Office.
