About the Role

We are seeking a highly skilled Geriatric Social Work Specialist to join our interdisciplinary team at Fraser Health. As a member of our team, you will work closely with older adults and their families to provide geriatric social work expertise and support.

Key Responsibilities
  • Conduct comprehensive biopsycho-emotional and social assessments for older adults and their families.
  • Develop and implement client care plans in collaboration with the interdisciplinary team.
  • Provide education and information sessions to clients, family members, and staff on various topics related to aging and health.
  • Consult with other healthcare professionals to ensure high-quality care and services.
  • Prioritize workload and manage competing demands in a fast-paced environment.
Requirements
  • Masters of Social Work degree from an accredited institution.
  • At least two years of recent clinical experience working with older adults in a healthcare setting.
  • Current registration with the BC College of Social Workers.
  • Valid BC driver's license and access to a personal vehicle for business purposes.
