About the Role
We are seeking a skilled Geriatric Social Worker Specialist to join our team at Fraser Health. This is a dynamic and challenging role that requires a strong understanding of gerontology and social work practices.
Key Responsibilities:- Client Assessment and Care Planning: Conduct comprehensive biopsycho-emotional and social assessments for older adults and their families, developing and implementing individualized care plans in collaboration with interdisciplinary teams.
- Education and Training: Design and facilitate educational programs for clients, family members, and staff on topics such as chronic disease management, mental health conditions, and self-advocacy.
- Clinical Practice and Research: Participate in quality improvement activities, contributing to the development of program standards, procedures, and protocols. Engage in research projects related to gerontology and social work, collaborating with interdisciplinary teams to collect and analyze data.
- Advocacy and Support: Promote self-management and self-advocacy among clients, facilitating access to community services and resources as needed. Act as a client and family advocate, ensuring their voices are heard and respected.
- Administrative and Reporting Duties: Complete required reports, records, and workload statistics, maintaining accurate files and records in accordance with established policies and procedures.
Requirements:
- Education: Master's degree in Social Work from an accredited institution.
- Experience: Two years' recent experience working directly with older adults and their families in a healthcare setting or equivalent combination of education, training, and experience.
- Licensure: Current full registration with the BC College of Social Workers.
- Skills and Abilities: Demonstrated knowledge and understanding of counseling and therapy skills, clinical practice models, and evidence-informed gerontological practices. Excellent communication and interpersonal skills, with the ability to work independently and as part of an interdisciplinary team.
Compensation: $83,111 - $106,400 per year, depending on experience.
