Job Summary:
This is an exciting opportunity for a motivated and experienced leader to join our team as a Primary Care Network Development Lead. Reporting to the Director, Primary Care Networks, you will be responsible for the development of primary care networks and implementation of team-based care in patient medical homes.
Key Responsibilities:
- Develop and implement plans to advance the development and implementation of patient medical homes and primary care networks.
- Provide management and coordination of NSDFP activities related to the establishment of Primary Care Networks and Patient Medical Homes within respective PCNs.
- Ensure PMH/PCN goals and priorities are aligned with the North Shore's PCN strategic objectives, desired future state for primary care, funding parameters, and MOH policy directives.
- Establish relationships with physician practices and clinics to support their transition to patient medical homes providing interdisciplinary team-based care.
- Oversee the placement and orientation of nurses and allied health into patient medical homes and work with teams to successfully implement interdisciplinary care.
- Participate in the recruitment of VCH clinical staff for patient medical homes and provide leadership, guidance, coaching, and support to staff in partnership with the VCH manager for primary care.
Requirements:
A Master's Degree in a relevant discipline with minimum five years' progressive leadership experience, including experience managing and/or supervising interdisciplinary healthcare programs and/or healthcare projects.
Excellent communication, consultation, and relationship building skills to function within a complex interdisciplinary environment, including demonstrated experience working effectively with physicians.
Salary:
$94,900 - $111,700 per year.
