Primary Care Nurse Role:
We are currently seeking a highly skilled Primary Care Nurse to join our team at Vancouver Coastal Health.
The successful candidate will work collaboratively with our interdisciplinary team to provide comprehensive care to clients with complex health needs.
This role requires a high level of autonomy and the ability to work effectively in a fast-paced environment.
The ideal candidate will have a strong commitment to client-centered care and a passion for working in a community-based setting.
Responsibilities:
- Provide primary care services to clients with complex health needs
- Collaborate with the interdisciplinary team to develop and implement care plans
- Conduct physical and mental health assessments, identify problems and address issues or variances from the plan of care
- Act as a resource to the team by providing information and nursing support based on nursing theory and practice related to clients with complex health care needs
- Establish, maintain and enhance therapeutic relationships based on respect
Requirements:
- Current practicing registration as a Registered Nurse (RN) or Registered Psychiatric Nurse (RPN) with the British Columbia College of Nurses and Midwives (BCCNM)
- One (1) year of recent related experience or completion of a full final clinical placement in a community setting, or an equivalent combination of education, training and experience
- Valid BC Driver's License and access to a personal vehicle for business-related purposes, as required
