Job Summary

We are seeking a highly skilled and experienced Nurse Practitioner to join our team at the Thunder Bay Regional Health Sciences Centre. As a key member of our interprofessional healthcare team, you will provide high-quality care to patients with kidney disease, focusing on hemodialysis management.

Key Responsibilities
  • Provide advanced clinical assessment, diagnosis, and treatment of patients with kidney disease
  • Develop and implement individualized care plans in collaboration with the healthcare team
  • Lead and participate in interdisciplinary care teams to improve patient outcomes
  • Collaborate with nephrologists and other healthcare professionals to ensure comprehensive care
  • Conduct health education and promotion activities to support patients and families
  • Participate in quality improvement initiatives to enhance patient care and safety
Requirements
  • Current registration as a Registered Nurse in the Extended Class with the College of Nurses of Ontario
  • Membership in the Nurse Practitioners' Association of Ontario (NPAO)
  • Successful completion of graduate studies in nursing
  • Minimum 3 years of recent Nurse Practitioner experience in an acute care setting
  • Knowledge of and adherence to the Hospital's Code of Conduct
  • Demonstrated commitment to the principles of Patient and Family Centred Care
  • Clinical knowledge and expertise in chronic kidney disease management
  • Previous experience in adult education and program development, implementation, and evaluation
  • Strong leadership and interpersonal skills
  • Ability to work collaboratively within an interprofessional team
  • Excellent communication, negotiation, and conflict resolution skills
  • Effective group facilitation skills
  • Strong organizational, critical thinking, and problem-solving skills
  • High level of initiative and self-direction
  • Knowledge of local and regional issues related to chronic kidney disease care
  • Ability to plan, manage, and implement change effectively
  • Demonstrated commitment to continuing professional education
  • Proficient in computer applications and software such as Microsoft Office, PowerPoint, and Excel
