Job Summary

We are seeking a highly skilled and experienced Nurse Practitioner to join our team at St. Joseph's Care Group. As a Nurse Practitioner, you will play a critical role in providing high-quality, patient-centered care to vulnerable populations in our community.

Key Responsibilities
  • Provide comprehensive physical and mental health assessments and treatment plans for clients
  • Work collaboratively with the GAPPS tri-partner agency team to ensure seamless care coordination
  • Develop and maintain strong relationships with community partners and stakeholders
  • Participate in ongoing professional development and education to stay current with best practices
  • Contribute to the development and implementation of program policies and procedures
Requirements
  • Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (Primary Health Care or Adult)
  • Several years of experience in community health and/or mental health
  • Membership in professional association e.g. NPAO
  • Advanced clinical experience, skills, and judgment in mental health
  • Superior ability to communicate effectively and concisely, both orally and in writing
  • Strong knowledge of community and regional services
  • Ability to function and contribute as an effective team member
