Position Overview
The Nurse Practitioner for Long-Term Care Outreach will play a crucial role in providing high-quality nursing services to diverse patient groups within the community setting. This role is under the supervision of the Director of Trauma Program, Emergency & Critical Care Services.
- Must hold a valid registration as a Registered Nurse (EC) with the College of Nurses of Ontario
- Completion of an accredited Nurse Practitioner educational program
- Current Basic Cardiac Life Support (BCLS) certification is required
- Experience in managing both acute and chronic health issues in geriatric patients
- Certification in geriatric nursing is preferred
- Demonstrated ability to work autonomously as well as collaboratively within a multidisciplinary team
- Excellent communication skills and ability to build rapport with patients
- Strong critical thinking, problem-solving, and conflict resolution skills
- Proficient in physical assessments, intravenous therapy, aseptic techniques, and management of central venous lines
- Ability to integrate adult learning principles and evidence-based practices into clinical care
