Job Title: PACT Service Coordinator

At Shared Health, we are committed to providing high-quality mental health services to our community. We are seeking a highly skilled and dedicated PACT Service Coordinator to join our team.

Job Summary

The PACT Service Coordinator will be responsible for assessing, planning, and coordinating treatment, rehabilitation, and support services for program participants. This role requires a strong understanding of psychosocial rehabilitation and recovery principles, as well as excellent communication and interpersonal skills.

Key Responsibilities
  • Assess and plan treatment, rehabilitation, and support services for program participants
  • Coordinate services with multidisciplinary teams, including psychiatrists, nurses, and other healthcare professionals
  • Develop and implement individualized treatment plans
  • Provide education and support to program participants and their families
  • Collaborate with community agencies and organizations to provide comprehensive services
Requirements
  • Minimum four years' related community experience working with people with serious and persistent psychiatric disabilities
  • BSW, OT, PT, BSc PN, RPN, BN, RN, SLP, RD, or related health/human service degree required
  • Vehicle and driver's license required
  • Working knowledge of the principles of psychosocial rehabilitation and recovery
  • Skills in assessing, planning, development, and coordination of services
