Job Summary

The PACT Service Coordinator is a key member of the multidisciplinary team responsible for providing comprehensive support services to individuals with serious and persistent psychiatric disabilities.

Key Responsibilities
  • Assess and develop individualized treatment plans in collaboration with the clinical team.
  • Coordinate and provide services to promote recovery, rehabilitation, and community integration.
  • Work closely with the team leader and psychiatrist to ensure effective clinical supervision and guidance.
  • Develop and maintain relationships with community agencies and service providers to ensure seamless service delivery.
  • Provide education and support to individuals and their families to promote mental health and well-being.
Requirements
  • Minimum four years' experience working in community mental health settings.
  • BSW, OT, PT, BSc PN, RPN, BN, RN, SLP, RD, or related health/human service degree required.
  • Registration with relevant regulatory bodies and a valid driver's license are essential.
  • Working knowledge of psychosocial rehabilitation principles and practices is required.
Preferred Qualifications
  • Experience working with individuals with addictions and co-occurring disorders.
  • Strong networking and agency relation skills.
  • Excellent organizational, communication, and interpersonal skills.
