Position Overview
As a Mental Health Service Coordinator, you will play a vital role in supporting individuals on their journey towards recovery. Under the guidance of a clinical supervisor and psychiatrist, you will assess, plan, develop, and coordinate treatment, rehabilitation, and support services tailored to each participant's unique needs.
Responsibilities
- Conduct thorough assessments to understand individual needs and goals.
- Develop personalized care plans in collaboration with participants, families, and the multidisciplinary team.
- Coordinate and facilitate access to a range of services, including therapy, medication management, and community support programs.
- Provide ongoing support and monitoring to ensure treatment effectiveness and progress.
- Collaborate effectively with other healthcare professionals, social workers, and community partners.
Qualifications
- Minimum four years of experience working with individuals experiencing mental health challenges in a community setting.
- Bachelor's degree in Social Work, Occupational Therapy, Physical Therapy, Nursing, Speech-Language Pathology, or a related field.
- Current registration with the relevant professional regulatory body.
- Valid driver's license and access to a reliable vehicle.
