Mental Health Nurse Specialist

Job Summary
Fraser Health is seeking a skilled Mental Health Nurse Specialist to join our team. As a key member of our interprofessional care team, you will provide treatment and care management services to individuals with mental illness and concurrent disorders.
Responsibilities
- Coordinate care for clients by collaborating with the interprofessional care team, performing intake functions, conducting mental health assessments, developing comprehensive treatment plans based on client needs, administering prescribed medications, and monitoring clients' mental state.
- Plan and provide direct nursing care to clients, including reviewing client/family history, conducting physical/psychosocial assessments, assessing vital signs, and implementing other treatment modalities as required.
- The nurse works from a strengths-based model, using motivational interviewing and engagement techniques to develop individualized care plans and assist clients to access appropriate services in the community.
- Establishes, maintains, and enhances therapeutic relationships with clients and their informal support network by engaging stakeholders in treatment planning and evaluation, providing support and follow-up planning, promoting self-care, and empowering clients to improve their quality of life.
- Provides treatment services, including crisis intervention, individual counseling, and education, according to various theoretical models to clients and their informal support network as required.
- Initiates and participates in case conferences/meetings with clients, family, service providers, and members of the interprofessional team for identification of client mental health issues, coordination, emergent issues, and discussion of ongoing follow-up care.
- Acts as a client advocate to protect and promote clients' rights to autonomy, respect, privacy, dignity, and access to information.
- Collaborates with members of the interprofessional care team, including physicians, primary care providers, clinical nurse educators, and other healthcare professionals/providers, in the identification and resolution of issues, adjustments to care plans, and changes in client condition.
- Maintains a safe environment for clients, visitors, and employees by adhering to established safety and emergency practices and procedures and reporting any risks and anomalies to the Team Coordinator.
- Documents assessments, care plans, observations, interventions, client/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
Requirements
- Graduation from an approved School of Nursing or Psychiatric Nursing and two years of recent related experience providing treatment services to individuals with mental health and/or concurrent disorders or an equivalent combination of education, training, and experience.
- Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Valid Class 5 BC Driver's License and access to personal vehicle with appropriate insurance coverage.
Estimated Salary Range: CAD $55,000 - $73,000 per year
