Job Summary

Prairie Mountain Health is seeking a highly skilled and dedicated Nurse Practitioner to join our team. As a key member of our primary care team, you will provide comprehensive and patient-centered care to individuals and families in our community.

Key Responsibilities
  • Assessment and Diagnosis
    • Conduct thorough health assessments to identify health concerns and develop treatment plans.
    • Diagnose and manage acute and chronic illnesses, including ordering diagnostic tests and prescribing medications as needed.
  • Consultation and Collaboration
    • Work collaboratively with healthcare teams to provide coordinated and holistic care to patients.
    • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions.
  • Prescribing and Medication Management
    • Prescribe medications and monitor patient response to treatment.
    • Provide education and support to patients on medication use and management.
  • Screening and Diagnostic Tests
    • Order and interpret diagnostic tests, including lab results and imaging studies.
    • Consult with specialists as needed to ensure accurate diagnosis and treatment.
  • Injury and Illness Prevention
    • Develop and implement health promotion and illness prevention strategies for individuals and communities.
    • Provide education and counseling to patients on healthy lifestyle choices and disease prevention.
  • Professional Responsibilities
    • Document patient care in a timely and accurate manner.
    • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements

To be successful in this role, you will require:

  • A Master's degree in Nursing with a focus on Primary Care or a related field.
  • Current registration as a Registered Nurse with the College of Registered Nurses of Manitoba.
  • Current certification as a Nurse Practitioner with the College of Registered Nurses of Manitoba.
  • Experience working in primary care and community health settings.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
