POSITION SUMMARY
In the realm of Community and Long-Term Care, the Nurse Practitioner utilizes advanced nursing and medical principles to deliver clinical services, including the ordering and interpretation of diagnostic evaluations, medication prescriptions, and execution of minor invasive procedures. Key responsibilities include promoting healthy lifestyles, preventing illness or disability, managing both acute and chronic health conditions, and collaborating within multidisciplinary teams. The Nurse Practitioner offers comprehensive primary health care services to clients and their families.
This role operates in alignment with the Vision and Mission of Prairie Mountain Health, adhering to Primary Health Care Principles, the Registered Nurses Act - Extended Practice Regulation, Standards of Practice for Registered Nurses on the Extended Practice Register, Competencies for the Registered Nurse (Extended Practice), and the Code of Ethics for Registered Nurses.
The Nurse Practitioner may also provide mobile services through the Mobile Primary Care Unit and to adjacent communities.
RESPONSIBILITIES:
Overview:
Assessment, Diagnosis, and Treatment
- Evaluate, diagnose, manage, and monitor health concerns in the context of client experiences and health determinants.
- Conduct thorough and focused health assessments.
- Order diagnostic procedures such as x-rays and lab tests in accordance with professional standards and Prairie Mountain Health policies.
- Communicate health findings to clients, discussing outcomes and prognoses.
- Continuously assess client responses to treatment plans and make necessary adjustments.
- Perform advanced therapeutic interventions, including minor surgical and invasive procedures, essential for effective clinical management.
- Adopt a long-term perspective on client care consistent with family-centered nursing principles to ensure continuity.
- Encourage client independence and self-care based on their capacity.
- Implement harm reduction strategies.
Consultation and Collaborative Practice
- Engage with family physicians and healthcare team members to provide coordinated, comprehensive services.
- Serve as a clinical expert/resource to enhance client care delivery.
- Exhibit expert skills in both formal and informal consultation and collaboration with all healthcare professionals and clients.
- Promote collaborative practice among team members.
- Demonstrate client-centered care emphasizing an interdisciplinary approach to holistic care delivery.
- Refer clients to appropriate programs and services as required.
- Facilitate team and client care conferences as necessary.
- Apply theories of family dynamics and interactions in managing care for individuals and families.
Prescribing Medications
- Utilize pharmacological principles in selecting and prescribing medications as part of treatment plans.
- Monitor client responses to drug therapy and adjust prescriptions as needed to achieve desired outcomes.
- Educate clients on medication use, providing support and guidance.
Screening and Diagnostic Evaluations
- Order and interpret necessary screening and diagnostic tests based on clinical reasoning and evidence-based practices.
- Consult with specialists for clarification on diagnostic interpretations when needed.
- Follow up with clients regarding test results and develop appropriate care plans.
Injury and Illness Prevention
- Adopt a population health approach in planning and implementing health promotion and illness prevention strategies.
- Provide education and counseling to clients and their families.
- Identify the need for health promotion and prevention strategies for individuals and groups, implementing as necessary.
- Focus on wellness promotion and disease prevention.
- Conduct risk assessments related to injury and disability prevention, collaborating with clients and service providers to develop health plans.
Professional Responsibilities and Accountability
- Document clinical care accurately and timely in accordance with Prairie Mountain Health policies.
- Complete required statistical data and reports.
- Demonstrate cultural competence in practice.
- Engage in research relevant to nursing practice.
- Utilize research findings to inform clinical practice.
- Provide clinical supervision and mentorship to nursing students and other healthcare professionals.
- Promote the role of the nurse practitioner to clients and other professionals.
- Develop an education plan to maintain clinical competency.
- Commit to ongoing professional development through education and training.
- Propose and update relevant policies and procedures in consultation with management.
