Counselor and Case Manager

About the Role:
Rehab First Inc. is seeking a skilled Counselor and Case Manager to join our multidisciplinary team in Ontario.
Job Description:
We are looking for a highly motivated individual with a Master's degree (MSW) and current registration in good standing with the Ontario College of Social Workers and Social Service Workers. The ideal candidate will have experience in a healthcare setting, preferably in the community, and be familiar with relevant legislation such as the Health Care Consent Act and Mental Health Act.
Responsibilities:
- Psycho-social Assessments
- Selective assignment of Case Management responsibilities
- Development of OCF-18 plans under Ontario's Statutory Accident Benefit regulations
- Provision of counseling services in various settings, including in-person, via telephone, and cyber counseling
- Extensive documentation and report writing
- Ongoing collaboration with other medical and rehabilitation professionals
- Opportunity to work with adults and children
- Individual, couple, and family counseling
- Career and vocational counseling and assessments
Requirements:
- Master's degree (MSW)
- Current registration in good standing with the Ontario College of Social Workers and Social Service Workers
- Experience in a healthcare setting, preferably in the community
- Familiarity with relevant legislation, such as the Health Care Consent Act and Mental Health Act
- Superior communication, problem-solving, time management, and documentation skills
- Self-motivated and demonstrated ability to manage a busy and varied caseload
- Valid Driver's License and auto insurance and access to a reliable vehicle
- Completion of a satisfactory Police Vulnerable Sector Check within the last six months
